CrescentCare will only process requests it receives using this form when the request is initiated by a patient for his/her/their own record and submitted with a photo ID attached. Legal guardians may also submit requests but may be required to provide additional proof of patient-guardian relationship. All other requests must be submitted in person at any CrescentCare location, mailed to 1631 Elysian Fields Avenue, New Orleans, LA 70117, or faxed to 888-736-9806.

CrescentCare may charge reasonable fees for copies per Louisiana Revised Statute 40:1165.1 and requests may take up to 14 days.
